The centerpiece is the causal chain: a why-ladder that starts at the symptom you described and works down, one "but why is that true" at a time, to the real root cause — including whether that root cause sits at the business-process level or the founder-decision level.
Read the causal chain top to bottom, not just the summary. The bottom rung is the thing actually worth fixing — the rungs above it are downstream effects, not separate problems.
